The positive effects of light to moderately intensive physical activity during pregnancy are not in question. Corresponding exercise recommendations can be found in national and international guidelines, which show a relatively solid evidence base. However, there are large knowledge gaps with respect to training recommendations for ambitious amateur sportswomen and elite athletes. Therefore, among both athletes and their supporting staff there is a large degree of uncertainty, which sports activity and to what extent it can be maintained safely without risk to the mother and her unborn child. This case report presents the evolution of physiological and psychological health parameters of a former sedentary and physically inactive nursing student during an 18 months period (three academic semesters), during which she first took part to a one-semester institutional physical activity (PA) program offered by her University, before being selected to participate in relay to the Race Across America (RAAM) with a team of the University. The four months before the RAAM, she followed a cycling specific training program. After the RAAM, she was followed-up the next eight months.

Heart failure is a clinical syndrome with different etiologies and phenotypes. For all forms, supervised exercise training and individual physical activity are class IA recommendations in current guidelines. Exercise training can start in the hospital, immediately after stabilization of acute heart failure (phase I). After discharge, it can continue in a stationary or ambulatory prevention and rehabilitation program (phase II). Typical components are endurance, resistance and respiratory training. Health insurances cover costs for three to six months. Patients with implantable cardioverter defibrillators or left ventricular assist devices may train in experienced centers.
